I think the problem is death penalty is negated by killing monsters. However, if you're already too weak (I don't know how, but one of our tanks was at 83 hp max) then you won't be killing much.
Therefore, I believe that the majority of mission failures will be cured if they simply made death penalty go away after time. Many parties are patient enough to wait for the monsters to stop guarding their bodies, but with permanent -60% death penalty, we don't even have an option to wait; we cannot go any further.

I disagree on that one. I've been through the game three times, currently on my fourth run (with three different characters), and I'm pretty sure I know alot about most of the missions - more than the average player. If I'm in a mission, I generally give alot of advice, tell them about what they should keep in mind, what kind of monsters are ahead, show them spawns and behaviour (mobs are generally behaving very predictable), etc. In almost every group though, there is someone not listening. If I say 'don't rush in', you can bet that at least one (most of the times a warrior ^^) is doing exactly that - bang, group dead. I don't start insulting ppl (I never do that ingame, no matter the circumstances), but I start getting mildly ironical after that happens. And I don't think that's beyond what is reasonable, because without this kind of humour I'd probably already been driven insane by ppl not working in a team.

The community needs a serious overhaul...There should be a 'Nice people' district, and then a '****wad' district. Anyone seen in the wrong district will promptly be kicked out.

Yeah, the least they could do is replace players with a NPC that has the same skills/profession.

Everybody that's AFK for more than 10 minutes in an outpost should be permanently banned.
